The Unfiltered Truth
We had a 40-minute outage caused by a config change that three people approved. The postmortem doc was titled "Nadia incident — root cause". Blameless culture was on the careers page. It was not in the doc. I finished the on-call rotation, then handed the pager to my manager and asked him to carry it for a week before deciding if I was the problem.
